These Notes are very important. Please read them carefully along with the table of benefits, as they explain more fully the benefits that are available to you and apply to each person covered under your membership.

Note 3 - Other benefits

Note 3a: Private ambulance

If you need private day-case or in-patient treatment for which you are covered under your membership, and it is medically necessary for you to travel by ambulance, we pay up to a total amount of £Sterling300 or US$600 or €Euros450 as applicable each membership year for travel by local road ambulance only:

* from your home or place of work to hospital

* from hospital to home

* between hospitals

* between an airport or seaport and hospital.

Note 3b: Family doctor treatment and prescribed drugs and dressings

We will pay 80% of the amount that you are charged up to a total amount of £Sterling400 or US$700 or €Euros600 as applicable each membership year for treatment that you receive from your family doctor. This includes the costs of drugs and dressings prescribed for you by your family doctor for treatment.

Note 3c: Emergency dental treatment

We will pay 80% of the amount that you are charged up to a total amount of £Sterling400 or US$700 or €Euros600 as applicable each membership year for eligible emergency dental treatment that you receive each membership year. By eligible emergency dental treatment we mean emergency dental treatment consisting of:

* dental examination

* radiography (for example an X-ray)

* extraction of a tooth (or tooth root)

* stopping abnormal heavy bleeding (haemorrhage)

* cutting into an abcess

* dressing a tooth root canal

* prescribed antibiotics

* re-cementing a crown bridge or inlay

* adjustment or repair of a denture

* construction and fitting of a temporary crown

* call-out charge

We will only pay for eligible emergency dental treatment that you receive during your first visit to your dental practitioner for each separate dental emergency. We will not pay for follow-up visits for emergency dental treatment relating to the same dental injury or condition.
